Demise : Take Actions ?!?

In case of the death of an Air Veteran, the following actions are required to be taken :

Inform the Adjutant (Adjt) of the nearest Air Force Station (AFS) (Within Municipal limits), will help with the funeral arrangements. The Next-of-Kin (NOK) will be eligible for a grant of INR 7.5 K for the Dignified Last Rites. 

The claim needs to be made to the nearest AFS with

(a) A written Application by the NOK

(b) A copy each of the Death Certificate (DC) / Pension Payment Order (PPO) / Canteen Stores Department (CSD) Card duly attested by a Gazetted Officer /Sarpanch of the Village Panchayat. 

(c) For assistance, Adjt / Station Warrant Officer (SWO) / Sadbhavana Cell of the nearest AFS may be contacted. 

Obtain two ink signed copies of Medical Certificate specifying the cause of death from the Hospital or the authorized Medical Practitioner - One for Authority at the Cremation/Burial Ground who issues Cremation/Burial Certificate while the other for Registrar of Deaths and Births who issues Death Certificates. 

Listed below are the addresses where the NOK must share the information (By Hand / Speed or Regd Post / Courier)

(a) Secretary, IAF Benevolent Association (IAFBA), AFGIS Building, Subroto Park, New Delhi - 10 (Enclose Original copy of DC) 

(b) Secy, AFGIS, AFGIS Building, Subroto Park, New Delhi - 10 (Enclose Original copies of DC, Post Retirement Insurance Coverage - PRIC and Nomination Form) 

(c) Secy, AFWWA, Santushti Shopping Complex - NWC, Opp Hotel Ashok Samrat, Race Course, New Delhi - 03 (Enclose Original copies of DC / AFWWA Membership Card, only if life Member )

(d) Dte of Air Veterans (DAV), Air HQ (SP), I Floor,  SMC Building, Subroto Park, New Delhi - 10 (Enclose Original copy of DC) 

(e) Secy of the Air Force Association (AFA) (your nearest Branch) or the AFA Head Office (Enclose Original copy of DC).  Life Membership No. is mandatory to claim the AFA Grant.

Send the Documents separately to each of the above addresses as applicable. 

A close relative should apply for issue of the DC within 15 days of the death of the Pensioner. Obtain at least 20 ink signed and equal number of Photostat copies duly attested by a Class I Officer. 

Write to the Pension Paying Bank (Pension Disbursing Authy - PDA), intimating them of the demise of the Pensioner, asking them to discontinue the Pension of the Pensioner and start payment of the Family Pension to the Spouse / NOK (Give name). 

Enclose ink signed DC and copy of the PPO with a Joint Photo of the Pensioner and Spouse / or NOK.  Also state PPO and pension SB A/C Nos. 

If the Pension A/C is a Joint A/C or the Spouse is a Nominee in it, then it is simpler to operate the same A/C for Family Pension. Otherwise a fresh SB A/C is to be opened in the same Bank.

The NOK is eligible for a Death Relief Grant, in one lump sum, from The Secy, Rajya Sainik Board, provided the Pensioner was registered as a Veteran with the Board. Pl apply with copies of the PPO / DC attested by a Gazetted Officer. 

Pl appear in person, deposit the Identity Card issued by the Zila Sainik Board (ZSB) of the District of residence and register yourself as the NOK of the Veteran with three stamp size photos and obtain an ID Card. 

The NOK is eligible for the Family Pension from the GOI. Pl intimate by a letter about the death of the Pensioner to the Bank from where he was drawing his pension with a copy of the original DC. PPO will be available with the bank and it would come handy, if you carry. The Bank may further advise about the Docs that would be required by them.

Pl visit your bank at the earliest.  Write separately to the Pension Sanctioning Authority, to Principal Controller of Defence Accounts (Pensions) - PCDA (P) to start Family Pension, on demise of the Pensioner and enclose ink signed copy of DC to PCDA (P), Draupadi Ghat, Allahabad - 14.

The NOK is entitled to a rehabilitation grant from the IAFBA under the Family Assistance Scheme (FAS). Pl intimate the death of the Pensioner thro' an application along with the original DC to the Secy, IAFBA.

(a) FAS 97: Applicable for those who were retired prior to 31 Mar 97 and died after 01 Apr 97. Death after retirement of Veteran who has served atleast 20 yrs.  Remark .... For 08 Yrs 

(b) FAS 09 : Applicable for those who were in service on 01 Apr 09 and died after that date. Death after retirement of Veteran who has served atleast 10 yrs. Officers Airmen NC(E) Remark ... For 08 Yrs  

An Ex-Gratia of Rs. 4,000 will be paid by the concerned Branch of AFA for Air Force Association (AFA) members only. A copy of the AFA Membership Card and DC is to be submitted to concerned branch. 

Ex-Servicemen Contributory Health Scheme (ECHS). The NOK are eligible to join ECHS if not covered already. ECHS form is available in the internet.  For further assistance call ECHS at Toll Free No 1800114115. 

If your spouse had taken a PRIC policy, you are eligible for a PRIC claim. Forward the following docs to Secy AFGIS, AFGIS Building, Subroto Park, New Delhi -10

(a) A copy of DC attested by a I Class Gazetted officer 
(b) Post Retirement Insurance Memorandum issued by the AFGIS in original and the approved copy of nomination
(c) A photo of the beneficiary, duly attested by Magistrate / Stn Cdr of the nearest AF Unit / Secy, ZSB, President / Secy AFA of respective Branch is to be affixed in the place provided for in the claim form “AFGIS - 222” (Available from AFGIS). The bank confirmation proforma (AFGIS - 229 on the reverse of AFGIS - 222) is to be attested by the Bank Manager. 
 
In case the NOK is not sure of these actions, send the info with necessary papers to the AFA Branch/HO, the needful will be done.
