The following documents are required to be
produced by a family pensioner for receipt of first payment after the death
of the spouse/family member in receipt of pension:

1.

Call letter sent to them by the DPDO.
|
2.
|
Non employment certificate/re-employment
certificate and Certificate for non-drawal of any other family pension in
respect of the deceased, issued by the Village/ revenue authorities.
|
3.
|
NBS Option Form for indicating the bank
branch. This should be got filled in and signed under the seal of the
respective bank branch.
|
4.
|
Three photographs duly attested by Gazetted
officers/ Secretary Zila Sainik Board.
|
5.
|
Pension Certificate (in case of
Civilians)/Discharge certificate (in case of PBORs) of the deceased.
|
6.
|
Pension Certificate issued to them by the
Pension Sanctioning Authorities (i.e., PCDA(P) Allahabad or PCDA(Navy) Mumbai
or CDA(Air Force) as the case may be).
|
7.
|
Identity Card issued by Govt Department or
any autonomous/semi-autonomous organisation (not applicable in case of
Commissioned Officers).
|
8.
|
Bank Pass Book of the deceased and the self.
|
