My dear  Retirees, 

Of late I came to know  that  the Retirees who want to  change  their  spouse  name, have no proper  records,  to justify their  request. 

For example, 

If a Retiree  wants  to  change  his spouse  name from Zulaika Begum to Zulaika  Khan, ( this happens after her marriage  with Mohammed  Khan) the Retiree may not have proper  Records,  to justify  his request. 

In these cases, kindly do the  following:-

1) The Retiree 's wife should submit a letter to the Gazatted Notification  Officer, to change  her  name  as Zulaika  Khan  instead  of  Zulaika  Begum. 

2) In that request  letter please don't  mention that she wants to change her  name  in connection  with  the  pension  pay order. 

3) you ask her to make a request  to  change  her  name,  since she  was advised to  change  her  name,  as per  numerology  advise given  to  her. 

4) In that request  letter,  her signature should be  attested  by a Gazatted  officer.
5) A Bank Manager  can  also  attest  her signature. 

6) At Chennai, this Gazette  Notification  Office is situated,  just opposite  to  our Union Bank of  India,  Mount Road Branch.

7)The above office is only for Tamilnadu  and for other  states,  you kindly  approach  the individual  state office. 

8) You can apply for more than one copy .

9) While submitting  your  request,  to change  the spouse  name, kindly  enclose  one original  copy also.

10) You can retain the rest of the original copies  for  your  records/for any other  purpose.
