LTA (Nomination for Life Time Arrears (FORM-A)), is required to be filled (Three copies) by all ESM and is to be submitted to their PDA (Pension Disbursing Agency i.e. Bank Branch from where Pension is being drawn) for further action and submission of one copy to their CPPC. Prepare four copies of LTA, submit three copies to the Bank and retain the fourth copy duly receipted by the Bank with Stamp, date and Signature of authorised person.
Submission of LTA is most important for the payment of any Arrears, which were payable to the ESM but were not paid during their life time and are now payable to nominee of ESM after the demise of ESM. This is a legal requirement to authorise the PDA/CPPC to pay any amounts due to ESM after his death to his/her nominee. LTA is additional requirement in addition to nominee authorisations incorporated in the PPO/ePPO/Discharge Book etc.
It is therefore, necessary for all ESM to submit LTA at the earliest to their PDAs. Copy of LTA (FORM-A) is posted by many members above.
