Change of date of birth of the wife of a former military man in military documents: 


Detailed process Discrepancies in date of birth on Adar Card, PAN Card, Voter Card and Military Documents i.e. H. Service Documents, Medical Certificates, ECHS Card, Relatives Card can create a very problematic situation over time . 

If the date of birth (DOB) of the wife/widow is not mentioned in the service record/PPO/discharge register or only age, year or wrong date of birth is mentioned and her Adhaar card and PAN If it does not match the card, apply for a change of her date of birth in the service records. If the wife's name in the documents/PPO/discharge register is incorrect, please correct her name and her date of birth at the same time.

 A) Condition:- 

1) Before changing the date of birth, please check whether the wife's name is recorded correctly  in the service file/PPO/discharge register. If not,  change your name and date of birth at the same time. 

2) If the change in date of birth makes the wife a minor on the date of marriage, the new date of birth may not be accepted by the registry office. Since 1978, the legal age for marriage for girls has been 18 years or older. 15 years before that.

3) If the difference between the previous DOB and New DOB is very large, more than 3 yrs, some Records Offices may not accept. 

B) DOCUMENTS REQUIRED:-

1) PERSONAL APPLICATION:
(a) In 3 original copies.
(b) Signed by ESM/Widow and Not Wife.
(c) Please give your correct and full Address with PIN Code number, current Mobile number and Email ID (if available) on top right corner of the Application. 
(d) Please keep adequate space at the bottom of the application for signature and stamp of Secretary ZSB.
(e) Please write, Rank, Name, Army Number, PPO Number of ESM. 

2) AFFIDAVIT.


(a) Submit in single ORIGINAL copy. 
(b) On a Non-Judicial Stamp Paper from Class -1 Judicial Magistrate/Notary.
(c) In English language.
(d) Signed by ESM/Widow as Deponent and not by WIFE. 
(e) Affidavit should NOT be more than 6 months old and preferably from nearest courts of your home location. 
(f) Rank, Name Service No. PPO No, Date of Retirement of ESM must be mentioned in first paragraph.
(g) Incorrect DOB of Wife (as per PPO/Discharge Book/Service Records) and correct DOB (given in Adhaar and PAN cards) are belong to same person must be mentioned.

3) ADHAAR CARD OF WIFE.


(a) One Photocopy must be given, attested by wife of ESM OR widow. 
(b) Name must be correct and matching with PPO and Service Records.
(c) DOB should be in full DDMMYYYY format.  

4) PAN CARD OF WIFE.


(a) Wife’s father’s name and DOB should be correct. 
(b) Photocopy must be  attested by wife/widow. 

5) BIRTH CERTIFICATE (BC).
(a) Issued by Registrar of Births & Deaths only.
(b) Mandatory, if DOB is after 1985. 

6) EDUCATION CERTIFICATE:

(a) 10th Board Certificate/Marksheet/Admit Card, having correct Name and DOB of Wife or Widow.
(b) Wife’s DOB, Name and father’s name should match with her other documents. 

7) MARRIAGE CERTIFICATE (MC).

(a) Marriage Certificate is compulsory as per Army HQ orders, but some Record Offices are not insisting, if adequate other supporting documents are available. If you have MC, you must give.

b) If Wife’s Birth Certificate and 10th Board Certificates are available, then Marriage Certificate may not be given.

(c) In Marriage Certificate, wife’s Name, full DOB and Father’s Name must be written. 
(d) Name and details of issuing authority must be mentioned. 


Note-1: Out of Birth Certificate, School Final Certificate and Marriage Certificate, one or two must be given.
Note-2: If None of these three Certificates are available, then a Certificate from Gram Panchayat or Municipality executive mentioning her Name, Father’s name, Address and DOB as per known sources such as Adhaar Card and PAN Card must be given. 

8) PPO.


(a) Must be given.
(b) Photocopies of original PPO, Corrigendum PPO, Disability PPO, if any,  all are to be attached. 
(c) To be signed by ZSB.  


9) DISCHARGE BOOK(DB).


(a) All pages must be given. 
(b) Widows will give Service Particulars Certificate (SPC) copy, if available. 
(c) To be self-attested by Secretary ZSB.
(d) DB/SPC to be signed by ZSB.  

10) Voter ID Card. 

If DOB of wife in her Voter ID Card is correct, then you can give the copy of that Card. 


11) ECHS CARD.


If 64 KB Medical Card has been issued to wife with correct DOB, then you must give photocopy of that Card, signed by wife. 


12) SELF DECLARATION FORM. 


(a) Signed by ESM/Widow. 
(b) Signed by Two ESM Witnesses. They must give details of name and address, and Army No.  
(c) To be Countersigned by OIC ZSB.

13) PDA FORM. 
(a) This is to be signed by ESM/Widows. 
(b) Signed by Bank Manager of Pension Account Bank. 
(c) Original round Stamp of Bank and Bank Manager are mandatory. 
NEWSPAPER ADS. NOT REQUIRED, IF ONLY DOB IS TO BE CHANGED.

Note:  If both NAME and DOB are to be changed/corrected together, then Newspaper Ads are  required. C) PROCESS:
1) Bring all your Documents along with all Original Documents to ZSB 
2) Show your Documents to Clk at Counter No 3/5 for verification. (All original documents will be returned to you after verification). 
3) If all okay, ZSB Secretary will sign and recommend your case. 
4) Only a copy of Application will be kept at ZSB. No other documents are required. 

5) One application copy, after signature of officer will be given to you as your Receipt Copy. Keep photocopies of all documents sent to Records Office, with you for future reference.
6) One Original Personal Application along with all supporting documents will be given to you. 
7) You Speed Post the set from Post Office to your Record Office. DO NOT SEND BY COURIER. 

Note: Please DO NOT BRING 3 SETS OF ALL DOCUMENTS. Only three copies of Personal Application and all other documents in ONE COPY each required. 
8) Record Office will publish PART-2 Order and send a copy to you by post and also one copy to ZSB, 
9) When received at ZSB, a list will be posted in ZSB Telegram Group.  

10) Currently, Entry of Part-2 order in your Discharge Books/Service Particulars Certificate (SPC) for Widows is NOT being done due to shortage of staff. Keep a photocopy of Part-2 Order in your Discharge Book. Will call you to ZSB for entry in Discharge Books, when possible. 

NOTE: THERE IS NO NEED TO APPLY TO RECORD OFFICE FOR ENDORSEMENT IN PPO AND ISSUE OF CORRIGENDUM PPO. PCDA HAS INTIMATED THAT, NO CORRIGENDUM PPO WILL BE ISSUED. ALL NAME AND DOB CORRECTIONS WILL BE DONE DIRECTLY IN THE SPARSH PPO, AFTER MIGRATION. 

11) If your Pension has been transferred to SPARSH, then check DOB of wife/widow in SPARSH PPO. If incorrect, then give request for Personal Data Verification (PDV), when allowed, directly into SPARSH website. 

12) If your Pension Account has not been transferred to SPARSH, then please wait, till it get migrated and then check. 

13) If Wife’s DOB is still incorrect in SPARSH PPO and Profile, then put up a complaint in SPARSH website and give Part-2 Order Copy. SPARSH will send your complaint to your Record Office and get the Part-2 Order verified. After that SPARSH will update/correct the DOB of wife in SPARSH PPO and will intimate you by SMS. This facility to complaint will start shortly in SPARSH. 

14) On receipt of SPARSH PPO with amended DOB of wife/widow, download and keep copy of PPO for future reference.  

15) After getting SPARSH PPO with Correct DOB of your Wife, there will be No problem to get Family Pension by your wife in future.
