🗂️ Documents Required for Renewal / Re-application

Type of Applicant Documents to Upload / Submit
Old Card Holder (Renewal of 16 KB / 32 KB Card) 1. Copy of PPO (Pension Payment Order)
2. Death Certificate (if ESM deceased)
3. Disability Medical Certificate (if applicable under PwD)
4. Copy of old ECHS Smart Card
Temporary Slip Holder 1. PPO Copy
2. Death Certificate (if ESM deceased)
3. Disability Medical Certificate (if PwD)
4. Copy of Old Temporary Slip
Loss of Old Card / Temporary Slip 1. PPO Copy
2. Death Certificate (if ESM deceased)
3. Disability Medical Certificate (if PwD)

📑 Key Points

  • 64 KB ECHS cards have replaced all earlier versions (16 KB / 32 KB) — mandatory after 31 Dec 2022.
  • Renewal applications are made online at www.echs.sourceinfosys.com.
  • Photograph and signature of all members must be uploaded.
  • Temporary Slips are valid for 6 months, extendable by the parent Polyclinic for 3 months at a time.
  • White Cards are issued for War Disabled / PwD / Battle Casualty beneficiaries (priority treatment).

📋 Next Steps

  1. Log in to echs.sourceinfosys.com.
  2. Select “Old Card Holder” as application type.
  3. Upload the required documents (as above).
  4. Verify data with Record Office; await verification SMS/email.
  5. Collect the printed 64 KB card from your Parent Polyclinic upon OTP verification.
